Late Cancellation Policy
Welcome to our Aloha family! We appreciate your trust in our practice, staff, and doctors. To provide the highest quality of dental care, we reserve the doctor’s and staff’s time especially for our patients. Therefore, we request 2 business days' notice for any changes or cancellations to your appointment.
The fees for a missed appointment or an appointment cancelled without 48 business hours’ notice are $75 per child for a regular cleaning appointment and $100 per child for a treatment appointment or a Saturday appointment.
Our office understands that sometimes urgent or emergency situations arise, and we will handle each circumstance on an individual basis.
We truly appreciate your courtesy of giving us sufficient notice if you have a conflict with your appointment and need to schedule a different day and/or time. It is our philosophy to continue to put our patients first and to make your child’s experience a positive one.

Financial Policy
We are committed to providing your child with the best possible dental care. We will be happy to answer any questions regarding our policy or fees at any time.
- All patients must complete our "New Patient Registration Form" before being seen by the doctor.
- FULL PAYMENT IS DUE AT THE TIME SERVICES ARE RENDERED.
- WE ACCEPT CASH, CHECKS, AND VISA/MASTERCARD.
- THERE WILL BE A $25.00 CHARGE FOR CHECKS RETURNED TO OUR OFFICE.
MINORS ACCOMPANIED BY AN ADULT: The adult accompanying a minor, their parents (or guardians), are responsible for FULL PAYMENT at the time services are rendered.
UNACCOMPANIED MINORS: The patient is to bring payment with them at the time services are provided. Non-emergency treatment will be DENIED unless charges have been pre-authorized to an approved Visa/Mastercard credit card, or paid by cash or check at the time services are rendered.
SEPARATED OR DIVORCED PARENTS: The parent accompanying the child is responsible for payment unless prior arrangements are made.
Regarding Insurance:
For all insured patients, payments are expected at the time services are rendered. We can assist you by filing the claim electronically with your insurance carrier or will provide you the necessary information for you to submit to your insurance carrier. Reimbursement from your insurance company will be made directly to you.
YOU ARE RESPONSIBLE FOR THE TIMELY PAYMENT OF YOUR ACCOUNT.
Failure to pay the outstanding balance within the first billing cycle will result in a $5.00 service charge.
Insurance is a contract between you and your insurance company. We are NOT a party to this contract. We may contact your insurance company to obtain benefits. Benefits obtained are subject to change by your employer or group policy. We will not become involved in disputes between you and your insurance company.
Notice of Privacy Practices
You have the right to:
- Transfer copies of your child's health information to another practice. We will mail your child's files for you.
- See and receive a copy of your child’s health information, with a few exceptions. Give us a written request regarding the information you want to see.
- Request an amendment or change to your child’s health information. Give us your request to make changes in writing. If you wish to include a statement in your child’s file, please give it to us in writing. We may or may not make the changes you request, but will be happy to include your statement in your child’s file. If we agree to an amendment or change, we will not remove nor alter earlier documents, but will add new information.
- In the event your unsecured protected health information is breached, we will notify you as required by law. In some situations, you may be notified by our business associates.
- Receive a copy of this notice. If we change any of the details of this notice, we will notify you of the changes in writing.
You may file a complaint with the Department of Health and Human Services, 200 Independence Avenue, S.W., Room 509F, Washington, DC 20201. You will not be retaliated against for filing a complaint. However, before filing a complaint, or for more information or assistance regarding your child’s health information privacy, please contact our Privacy Officer at (510) 848-6494.
This notice goes into effect as of September 25, 2015, and will remain in effect until we replace it.
Patient Communication
Voice Messages: I understand brief messages from the dental practice may be left on my home answering machine or with anyone who answers the telephone at my home unless I have provided the practice with alternate instructions for communication.
Email: Except for appointment reminders, we use secure methods to electronically communicate with our patients. Unencrypted email is not a secure form of communication. There is some risk that any individually identifiable health information and other sensitive or confidential information that may be contained in such email may be misdirected, disclosed to or intercepted by unauthorized third parties.
However, you may consent to receive unsecure email from us regarding your treatment. We will use the minimum necessary amount of protected health information in any communication. Our first email to you will verify your email address. I consent and accept the risk in receiving information via unencrypted email. I consent to receiving appointment reminders via unencrypted email. I understand the minimum necessary information is used in these reminders. I understand I can withdraw my consent at any time.
Cellphone: I consent to the dental practice using my cellphone number to call and text regarding appointments and to call regarding treatment, insurance and my account. I understand that I can withdraw my consent at any time.
